About the Job
We are recruiting to the newly formed Learning Disability Transitions/18-24 Team. This is an exciting time to join the newly formed team offering high quality and timely transition support to young people and their carers in accordance with the Care Act and principles of Preparing for Adulthood. At Newham our approach is to empower residents drawing on their experiences hobbies communities and networks to build care planning that recognises them as an individual.
As a qualified Social Worker with two years post qualifying experience you will hold a case load where you will implement short and long term interventions utilising a strengths based approach to ensure that independence and wellbeing are promoted in the community. Social Workers will routinely undertake social care assessments reviews and support planning mental capacity assessments Continuing Health Care decision support tools and safeguarding enquiries when necessary.

About You
We are looking for candidates who have strong assessment skills with demonstrable experience ofdelivering high quality interventions for residents their families and carers.
You will havesound understanding of Care Act interventions along with significant knowledge of Strengths Based Practice Mental Capacity Court of Protection and Safeguarding Adults includingsignificant knowledge of risk and the current research and practice in this.
You will also have had experience building and developing relationships with partners in the voluntary community and faith sector health.
Excellent communication skills are critical and you will have demonstrable experience of working within multi-disciplinary settings.
