Job Description
This position is for a Physical Therapist providing skilled home health services in Sonoma CA and surrounding areas. The therapist will manage a caseload of up to six patients daily performing patient assessments treatment planning and providing direct patient care. The role requires adherence to a standard productivity level and may include weekend shifts.
- Develop and implement individualized treatment plans.
- Conduct patient assessments and document progress in electronic health records (EPIC/Oasis).
- Manage a caseload of up to six patients per day maintaining a 30-point productivity standard.
- Maintain accurate and timely documentation in Oasis.
- Adhere to established safety protocols and infection control measures.
- Collaborate with other members of the healthcare team.
- Manage travel between patient locations within a 50-mile radius.
- Minimum of one year of skilled physical therapy experience.
- Basic Life Support (BLS) certification.
- Proficiency in Oasis electronic health record system.
- Valid drivers license car insurance and reliable transportation.
- Experience with home health documentation.
- One weekend shift required per contract.
- Facility may call off the therapist for up to one shift per pay period.
- Occasional travel outside of a 50-mile radius may be required.
- Travelers are expected to have their own transportation.
