Job Description
This role requires a Registered Nurse with at least two years of experience in a hospital home health or hospice setting. The RN will provide skilled nursing care case management and collaborate with an interdisciplinary team to care for patients facing age-related frailty and serious illness. The position requires strong communication and collaboration skills and proficiency in patient assessments medication administration and treatment planning.
