Job Summary and Responsibilities
**Home Health Physical Therapist **
Provides physical therapy services to patients/clients according to a written physicians plan of care. This may include but is not limited to: Assessing evaluating and updating therapeutic/rehabilitative/functional status and participation in the development of the total plan of care. Evaluates home environment. Assesses for muscle strength mobility gait ROM potential for rehab. Directing physical therapy treatment. Instructing patients/clients and families/caregivers in the use and care of therapeutic appliances. Determining priority needs for physical therapy. Reporting the patients/clients reaction to treatment or changes in condition to the physician.
Job Requirements
Required:
- Two years of appropriate experiences as a Physical Therapist
- Physical Therapist: CA (PT:CA)
- Drivers License: CA (AUTO:CA)
