What Youll Do in This Role:

Primary Responsibilities:
- Provide personalized one-on-one guidance to help members access healthcare wellness services and resources.
- Build strong relationships with members families and healthcare teams.
- Advocate for members to ensure timely high-quality care and address barriers.
- Conduct assessments to identify short- and long-term needs.
- Develop strategies and goals to meet members needs.
- Facilitate smooth transitions across the healthcare continuum.
- Monitor progress and adjust care plans as needed.
- Coordinate continuity of care through effective communication.
- Participate in after-hours calls and occasional home visits.
- Accurately record member interactions in the EMR system.
- Connect members with community resources to support aging in place.
- Deliver exceptional customer service and foster a positive member experience.
What Youll Need:
- High school diploma required.
- Valid drivers license and proof of insurance.
- Ability to travel frequently within the West Michigan area.
- Proficiency in medical terminology.
- CNA Medical Assistant or Med Tech certification preferred (not required).
- Experience working with the geriatric population strongly preferred.
