PRIMARY RESPONSIBILITY: The RN Clinical Supervisor will provide supervision to RN Case Management staff and Acute Care Management staff.
ESSENTIAL JOB FUNCTIOINS:
- Responsible for the clinical supervision training and evaluation of Registered Nurse Social Work Care Management and Admin Staff.
- Participates in the hiring yearly performance evaluations and termination of Acute Care Management staff.
- Facilitates and oversees day-to-day operations; insuring that appropriate systems are in place.
- Performs regular audits of cases
- Ensures program and staff compliance with all regulatory requirements as set forth by California Department of Aging.
- Insures high quality timely care and service delivery
- Implements and monitors multiple systems including records clinical documentation and personnel. Ensures that documentation is compliant with regulations policy and procedures.
- Organizes conducts care plan conferences and individual supervision as needed
- Facilitates the orientation and training of all new staff including joint home or virtual visits supervisory meetings and documentation review.
- Responsible for assessing the educational needs of staff developing and coordinating an education and training plan that assures a high level of skill and competency in the understanding of case management and community resources.
- In collaboration with Leadership and MSW Clinical Supervisors lead the development and implementation of curriculum for all Care Management staff
- Develops and maintains collaborative working relationships with community individuals HHAs PCAs groups and organizations; takes advantage of opportunities to educate the public about IOAs services.
- Responsible for remaining current with changes in community resources Medi-Cal and Medicare regulations and procedures that will impact the practice and direction of the program; acts as a resource for staff.
- Provides back-up coverage for clinical staff which could include home and/or virtual visits.
- In collaboration with the leadership participates in establishing monthly quarterly and annual goals and objectives relevant to the needs of the program.
- In collaboration with Program Manager and Clinical Supervisor conducts annual review and revision of program policies and procedures.
- Responsible for expanding personal knowledge understanding of care management family systems geriatrics adolescent relevant heath care issues.
- Understands and applies regulatory and procedural requirements as well as the policies and procedures of the Institute on Aging.
- Provides mentorship consultation and training for all new CM staff and interns.
- Assumes responsibility for some of the programs most challenging clients.
- Participates in program leadership meetings and takes on program development projects as assigned.
- Acts as liaison between MSSP CDA and providers.
- May direct and monitor additional smaller programs to adhere to contract guidelines and acceptable clinical standards of practice.
- Travel when necessary
- All other reasonably related responsibilities as assigned.
EDUCATION: RN BSN. Preferred: MSN Masters degree in nursing
BACKGROUND AND EXPERIENCE:
- 3-5 years experience working with frail elderly adolescents disabled adults and children acute care and/or Home Health required.
- Supervisory experience (preferred)
- Experience with and understanding of the medical and psychosocial problems
- Exceptional communication presentation skills and time management.
- Detail oriented and well organized.
- Demonstrates case management skills and experience in the community health care delivery system.
- Demonstrates ability to supervise care management staff and provide coaching / mentoring to staff.
- Demonstrates ability and willingness to take on program development projects.
- Good problem-solving skills and the ability to prioritize and multiple tasks.
- Computer literacy required (Excel Word).
- Ability to navigate multiple platforms and EMRs.
PHYSICAL & LANGUAGE REQUIREMENTS:
- Ability to lift transfer push/pull maneuver and reposition up to 25 lbs
- Ability to reach bend and walk
- Five finger manipulation
- Ability to comprehend complex written materials and verbal instructions
- Ability to speak and write English clearly
COMPENSATION:
Range: $ 125000-$132000/annual
