Job Description
The Case Manager Opportunity:
The Case Manager provides community-based case management for individuals diagnosed with mental health and or substance use disorders. The Case Manager coordinates care individuals enrolled in Comprehensive Community Services Community Support Program Childrens Long-Term Support or the Coordinated Services Team program by collaborating with a multi-disciplinary team. The population served includes individuals with a mental health and/or substance abuse diagnosis.
Compensation and Benefits:
- The starting pay range for this role is $26.50-$30.85 based on education and experience.
- This job is full-time and in person working 8:00am-4:30pm on weekdays.

- As a full-time employee of NCHC you are eligible for the Wisconsin Retirement System (WRS). The WRS is a hybrid defined benefit plan. It contains elements of both a 401(k) or defined contribution plan and a defined benefit plan. Participating staff contribute 6.95% and NCHC will match this at 100% each pay period!
What Qualifies You:
- Bachelors or Masters degree in human services field
- At least one year of case management experience.
- Ability to work in fast-paced environment.
- Person-centered service in all interactions.
- Valid drivers license and use of a personal vehicle.
- Attention to detail and organized.
What a Case Manager does at NCHC:
- Determine ongoing eligibility for Community Treatment programs via the approved state functional screen
- Facilitate the assessment and service planning process including the identification of consumer needs goals objectives strengths barriers interventions and their priority in the treatment planning process.
- Assess ongoing needs including crisis situations to determine treatment interventions and service provision.
- Ensure the service plan and service delivery for each client is integrated coordinated and monitored
- Provide services in adherence to the pillars of Community Treatment program philosophy which include being strength-based recovery-oriented person-centered flexible trauma informed and consistent with best practice.
- Complete documentation in compliance with Community Treatment standards agency policy and applicable regulations.
- Adhere to scheduled work hours with regular and predictable attendance.
