Overview
We are seeking an experienced Registered Nurse Case Manager to join our Phoenix-based home health this role you will provide high-quality patient-centered care while coordinating services across an interdisciplinary team. You will develop individualized care plans manage a dynamic caseload and ensure compliance with all regulatory and organizational standards. This position requires strong clinical skills exceptional communication and the ability to balance productivity with compassionate care.
Key Responsibilities
Care Coordination & Clinical Oversight
- Develop and manage individualized care plans that address patients medical functional and emotional needs.
- Serve as the primary point of contact for patients and families ensuring seamless care coordination.
- Conduct regular assessments adjusting interventions as patient needs evolve.
Operational Excellence
- Manage caseload to meet productivity and quality standards.
- Complete accurate timely documentation in the EHR to support compliance billing and care continuity.
- Track and analyze key performance indicators (KPIs) to improve outcomes.
Compliance & Quality Assurance
- Adhere to Medicare state and organizational standards.
- Deliver evidence-based care and monitor quality metrics to maintain high standards.
Patient Outcomes & Education
- Promote measurable improvements in health independence and quality of life.
- Educate patients and families on condition management medication adherence and self-care strategies.
Qualifications
- Associates or Bachelors degree in Nursing (BSN preferred).
- Active RN license in the state of practice.
- Minimum 2 years of clinical nursing experience; 1 year in home health preferred.
- Strong assessment care planning and patient education skills.
- Familiarity with Medicare/Medicaid home health regulations.
- Proficiency in EHR systems; home health software experience a plus.
- Ability to manage a dynamic caseload while maintaining quality and compliance.
Work Environment
- Primarily in patients homes within the assigned region with occasional office-based meetings or training.
- Fast-paced patient-focused environment requiring adaptability and organization.
