Job Description
BAYADA Home Health Care is seeking aClinical Case Manager (RN required) for our Towanda PA Pediatric / Adult Home Care office. This office provides best in class care to clients throughout Towanda PA and surrounding counties.

Responsibilities for a Clinical Case Manager:
- Lead inspire and motivate others to provide exceptional care.
- Visit our Pediatric/ Adult clients and conduct assessments in Towanda PA and other surrounding areas.
- Develop the care plan and oversee its execution.
- Supervise LPNs RNs and home health aides (HHA) staff in the field.
- Facilitate new hire orientation.
- Conduct competencies on new and tenured staff.
Qualifications for a Clinical Case Manager:
- Registered Nurse (RN) with 1 or more years of experience as an RN.
- Supervisory experience preferred.
