Job Description
We are seeking an experienced and dedicated Patient Care Manager to join our team in Norwood. This role is critical in ensuring the highest standards of patient care within a home health setting. As a leader in this field you will drive initiatives to improve patient outcomes and ensure that every individual receives personalized and compassionate care.
Key Responsibilities:
- Manage and oversee patient care services ensuring compliance with all regulatory standards.
- Lead mentor and support nursing staff fostering professional growth and enhanced patient outcomes.
- Collaborate with interdisciplinary teams to develop and implement individualized patient care plans.
- Ensure accurate and timely documentation of patient care activities and outcomes.
- Conduct regular audits of patient records and care plans to uphold quality assurance.
- Act as a liaison among patients families and healthcare providers to ensure seamless care coordination.
- Engage in quality improvement initiatives and participate in ongoing professional development.
Required Qualifications:
- A minimum of two years experience in a management or supervisory role within a home care setting is preferred.
- Strong knowledge of professional standards Medicare Conditions of Participation and compliance with federal state and local regulations.
Required Certifications and Licensures:
- Current licensure as a registered nurse in the state of practice.
