DescriptionWe are searching for a transition navigator someone who will routinely meet and lead adolescents and young adults with medical complexities and their families through the three stages of transition from pediatric to adult care (readiness handoff and transfer). The transition navigator work with the care team to ensure that the appropriate members of the care team teach adolescents and young adults and their familys skills important for self-managing a complex chronic illness and proactively navigating a healthcare system as adults.

Qualifications:
- High School diploma required
- Bachelors degree in social work psychology human services education communications pre-med or related allied health preferred.
- CHW- Community Health worker certification from TX Department of health and human services preferred.
- 3 years experience in healthcare setting required.
- Experience in transition medicine or other transition related experience preferred.
Responsibilities:
- Transition Readiness
- Assist in implementing the transition curriculum developed by the team based on local and national recommendations
- Assist with planning and participate in transition education events.
- Meet with patients and their families during the Handoff stage at least annually. The number/frequency of meetings will be determined by individual transition needs
- Follow up by email/telephone with patient and adult provider after initial adult appointment in order to assure patient transferred to adult care and is satisfied with transfer process
- Partners with community organizations involved in the interests of the pediatric population.
- Demonstrates positive and effective interpersonal skills when dealing with patients/families visitors peers and other health care team members.
