Job Description
Hourly Pay Range:
$40.45 - $62.70 - The hourly pay rate offered is determined by a candidates expertise and years of experience among other factors.Position Highlights:
- Position: RN Patient Navigator - Neurosciences
- Location: Arlington Heights IL
- Full Time/Part Time: Full Time 40 hours/week
- Hours: Monday-Friday 8am-4:30pm hours and flexible work schedules.
- Required Travel: No
What you will do:
- Conduct comprehensive assessments of patients medical social and psychological needs.
- Collaborate with healthcare providers to develop individualized care plans that address patients unique needs and goals.
- Identify potential barriers to care and work to address them proactively.
- Serve as a liaison between patients families healthcare providers and community resources to ensure a smooth transition of care.
- Monitor and manage patients progress throughout their healthcare journey.
What you will need:
- Bachelors Degree Nursing or Masters Degree Nursing required
- 2 Years of clinical nursing experience preferably in a hospital setting
- Certified Case Manager (CCM) - Commission for Case Manager Certification (CCMC) Certification in Case Management (CCM) or related field required
- Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Current Registered Nurse (RN) license in the state of Illinois. required
