Description

Want to join our Team We are currently looking for an inpatient and home hospice physician who is an experienced collaborative leader who works in concert with Hospice of the Chesapeakes (HOCs) medical staff. This physician models excellence during the entire course of care beginning at admission through case management and eventually transitioning caregivers into bereavement services.


Office Location: Waldorf MD

Coverage Area: Charles and Calvert Counties

Status: 40 hours per week

Schedule: 5 8-hour days and rotational overnight and weekend call

Salary: Competitive commensurate with experience education and credentialing

This physician exercises care plan oversight and ensures the highest quality hospice care is delivered at HOCs Charles Inpatient Care Center (CICC) facility in Waldorf Maryland where the position is based. This physician leads the corresponding regional interdisciplinary hospice team and may support palliative and population health initiatives as needed. They provide daily direct patient care in the CICC facility and bedside care where patients reside including all levels of billable and administrative visits. They align best inpatient practices with the agencys second inpatient care center in Pasadena Maryland.

This physician nurtures relationships with regional referral sources and provides education to internal and external learners including medical residents and APP students. They participate in HOC-related grant/research activities. They collaborate directly with referring physicians around symptom management and Medicare hospice benefit eligibility.

The inpatient and home hospice physician models excellence in all aspects of documentation and billing practices including the certification of terminal illness visit notes orders and death certificates. They complete daily documentation within 24 hours of a patient encounter. This physician participates in a pool to provide on call for after hours weekends and holidays.

Main Responsibilities:

  • Provides all aspects of direct patient care to inpatients and home-based hospice patients as assigned.
  • Educates the medical community about the value of HOC services in an effort to grow the organization and promote utilization of HOC services.
  • Assists in running interdisciplinary (IDT) meetings. Collaborate with team managers to ensure IDT meetings are productive comprehensively address patient/family needs and all team members are encouraged to participate.
  • Provides internal and external education.
  • Makes home/residence and inpatient care center visits daily. Provides patient/family medical support. Provides F2F visits for team patients if/as assigned.
  • Provides assistance to Medical Services leadership to ensure HOCs practice is compliant with organizational policy local state and federal laws regulations standards and requirements of regulatory and accreditation agencies (e.g. JC/DOH/CMS) including those governing professional practice and contractual arrangements (e.g. research grants).
  • Effectively participates in Quality/Performance Improvement (QI/PI) programs in collaboration with supervisor and/or project manager.
  • Confirms patient eligibility for hospice services in accordance with admission/recertification criteria and per hospice policy. This includes CTIs (certificates of terminal illness) patient orders and death certificates as assigned.
  • Supports the Chief Medical Officer and the Director of Medical Services in preparations for surveys administrative review and/or peer reviews and in maintaining standards and procedures which comply with licensure and accreditation requirements. Assists in implementing corrective action required as a result of survey/review findings.
  • Other duties as assigned.

Your Qualifications:

  • Graduation from an accredited school of medicine or osteopathy active Maryland physicians license state-controlled substance registration and unrestricted federal DEA license all required.
  • Two years of post-residency and/or fellowship practice experience preferred.
  • Fellowship training in Hospice and Palliative Medicine preferred.
  • Understanding of the philosophy of hospice and palliative medicine required. Knowledge of applicable federal state local regulations and statutes governing hospice operations and patient care and adequate professional development to maintain licensure.
  • Excellent oral written and interpersonal communication skills.
  • Initiative-taking customer service mindset.
  • Good clinical judgment in medial decision-making.
  • Comfort with technology including voice dictation EMR utilization and strong time management skills.
  • High degree of professionalism including honesty integrity and an appropriate level of patient confidentiality required.
  • Flexibility and willingness to undertake new responsibilities as assigned required.
  • Willingness to participate after hours in important agency events including working some evenings and weekends.

Valid drivers license and a good driving record. Reliable transportation a must.

Medicare enrollment.

Lift and move 50 pounds safely.

Compliant with health screening and vaccination requirements of organization and position including TB screening and influenza vaccination annually.
