Job Description
- Provide comprehensive evaluation diagnosis and treatment plans for patients with endocrine conditions.
- Manage patients with type 1 and type 2 diabetes including insulin management and diabetes education.
- Conduct follow-up visits and monitor lab/imaging results to adjust care plans as needed.
- Collaborate with endocrinologists dietitians and other specialists to ensure high-quality coordinated care.
- Educate patients and families on disease processes medication use and lifestyle changes.
- Participate in ongoing quality improvement and clinical development initiatives.
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA-C) in State Name or eligibility for licensure.
- Prior experience in endocrinology internal medicine or primary care is strongly preferred.
- Certification in diabetes education (CDE/CDCES) is a plus but not required.
- Strong clinical assessment communication and organizational skills.
- Passion for patient-centered care and education.
