How youll contribute
Join a compassionate collaborative Behavioral Health Team dedicated to transforming lives. As a Clinical Liaison youll provide licensed psychiatric care to patients on our Behavioral Health Unit(s). Your expertise will shape discharge planning psychoeducational programming and group therapy experiences that leave a lasting impact.
What Youll Do:
Coordinate intake and pre-admission screenings
Assess patient suitability for admission through clinical judgment
Educate patients families and healthcare professionals on rehab options and facility expectations
Serve as liaison with external case managers discharge planners and payors
Conduct face-to-face educational sessions with physicians and case managers
Perform market research and implement development strategies
Gather and secure patient resource and benefit information
Apply knowledge of developmental and aging processes to ensure appropriate care delivery
Qualifications and requirements:
- Masters Degree in Social Work
- Marriage and Family Therapy or equivalent Professional Counseling pending State requirements.
- MSW LCSW LPC-I LPC LMFT Independent Practice Scope Licensure in State of Practice; preferred.
- Minimum 1 year of experience in discipline; preferred.
