Job Description

How youll contribute
ASocial Worker (LSW)who excels in this role:
- Interviews and assesses patients and/or patients family caregivers and/or legal representatives.
- Determines prioritizes provides and/or arranges for needed internal and external services/interventions.
- Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote monitor and evaluate compliance with patients treatment plan.
- Assists with discharge planning and processes. Identifies appropriate resources including transportation housing healthcare and social/spiritual services and provides referrals as part of the discharge plan.
- Supervises and/or trains new staff students and interns.
- Provides individual family and group therapy.

What were looking for
Applicants should be a Board Certified Social Worker or related field (i.e. counseling) in the state (CSW LPCA LCSW) within 6 months of hire. Additional requirements include:
- Education: Bachelors required; Masters preferred
- Certifications: Handle With Care within 30 days of hire.
- Requires critical thinking skills decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
