Facility Name: Kindred Hospital Dallas Central
Schedule: PRN

How youll contribute
ASocial Worker who excels in this role:
Provides social service and discharge planning functions for the hospital.
Provides resources for inpatients outpatients Emergency Department patients and support to the Case Management team for psychosocial needs.
Maintains performance improvement activities with the department.

What were looking for:
Applicants should have a Bachelors Degree Required; Masters Preferred. Additional requirements include:
- Current Social Work license for the state of TX
- Basic Life Support (BLS) within 60 days of hire
Minimum 3 years work experience in similar field of social work.
