Job Description
Long-Term Care Pharmacist
Support patient recovery and independence through personalized medication plans.
Key Responsibilities:
- Review and monitor complex regimens in skilled nursing or rehab facilities.
- Coordinate care with therapists physicians and caregivers.
- Educate patients and families on medication goals and outcomes.
Qualifications:
- PharmD with LTC SNF or rehab pharmacy experience preferred.
- Compassionate communicator with strong organizational skills.
- Familiarity with Medicare Part D regulations.
