Job Title: Diabetes and Nutrition Educator
Wage range: $28.56-$51.85
Job Summary:
Maintain high standards of existing ADA accredited diabetes program.
Accountable for assuring diabetes instruction complies with national standards.
In accordance with professional standards and hospital policies and procedures responsible for conducting individualized needs assessments on persons enrolled in the diabetes and medical nutrition therapy patient education program.
Assessment of patients and development of plans of care on all aspects of Diabetes medical nutrition therapy.
Collaborating and communicating with team members.
Contributing to and participating in a continuous quality improvement process to measure diabetes education program and to identify and address opportunities for improvement.
Provide diabetes education and follow up to newly diagnosed and established adults with diabetes
Education and follow up to new insulin pump patients
Education and management of pregnant patients with Type I diabetes
In-Patient
Responsible for providing clinical nutrition services including nutrition assessment modified diet formulation self-management training and nutrition intervention to patients/ patient satisfaction quality care regulatory agency compliance and good public relations are achieved through the safe and efficient use of resources.
In-patient needs would cover inpatient diabetes education and rotating on-call for the weekends.
Education and/or Experience Requirements:
Bachelors degree in nutrition and Dietetics or Registered Nurse. Language skills Mathematical skills Reasoning Ability.
Licensure & Certifications Requirements:
Washington Dietitian Certification and National CDR Registered Dietitian (Commission on Dietetic Registration) Registered Nurse
