Job Description

Position Title: Foot Care Nurse

Division:ByWard Family Health Team (BWFHT)

Job Status:Part-Time (22.5hrs/week)

Job Reports to: Executive Director Family Health Team (FHT)

Salary: $28.14 - $34.27/hr ($32 924 - $40 096)

JOB SUMMARY

The Foot Care Nurse is responsible for providing quality foot care services to help patients eliminate foot pain gain mobility and maintain healthy feet. The Foot Care Nurse acts as a resource to clients and colleagues can work independently and within a team and is compliant with legislation and standards of practice. The Foot Care Nurse is a key member of the clinics Diabetes chronic disease management program.

ACCOUNTABILITY

Staff are expected to work in a professional manner demonstrating above average qualities such as courtesy tact and judgment with respect to patients fellow colleagues and professionals. All staff are expected to work in parallel with the organizations confidentiality agreement. As a member of our team you are responsible for bringing issues that affect the smooth operation of the organization to the attention of your immediate supervisor. It is your responsibility to be up to date with all organizational communication.


ESSENTIEL RESPONSIBILITIES

  • Conduct foot assessments/re-assessments develop treatment & management plans and maintain accurate client progress notes in the clinics CHR;

  • Maintain approved cleaning and sterilization techniques for all instrumentation and tools used;

  • Follow all infection control best practices and guidelines as set out by regulatory and advisory bodies including but not limited to the College of Nurses of Ontario and PIDAC-IPC;

  • Educate and advise patients about the prevention and care of morbid conditions relating to chronic diseases like diabetes HIV etc.

  • Ensure continuity of patient care through exchange of information and communication with the patient family members and other service providers who may be involved with the patient

  • Work as part of an interdisciplinary team making referrals and providing support to patients of ByWard FHT

  • Build relationships with various community partners

  • Act as a resource for the team in assessing other social agencies and supports

  • Receive referrals from medical and other healthcare practitioners and consult with these referring practitioners to provide timely and optimal care for their patients

  • Maintain program statistics for purposes of evaluation and research

  • Complete and maintain all necessary documentation as per act and professional standards

  • Participate in continuous professional educational opportunities


ESSENTIAL QUALIFICATIONS

  • Current Registered Nurse or Registered Practical Nurse certificate and registration with the College of Nurses of Ontario

  • Current professional liability insurance

  • Knowledge of wound care lower limb circulatory diseases and diabetes management

  • Minimum of two years clinical practice experience as a Foot Care Nurse including providing foot care services to people living with diabetes

  • Advanced and Diabetic Foot Care certificate is preferred

  • Current Level C CPR course or equivalent

  • Excellent interpersonal skills and a commitment to inter-professional care

  • Strong clinical and effective time management skills. Experience in a high-volume setting would be an asset.

  • Demonstrated ability to work independently and collaboratively as a member of an interdisciplinary team

  • Excellent health assessment skills clinical judgment analytical and problem-solving skills

  • Ability to work with an electronic health record system

ADDITIONAL INFORMATION:

Please note that related education/experience bilingualism and the ability to work in a fully paperless environment (EMR) are essential qualifications for this position.

REPORTING STRUCTURE (To whom the incumbent reports to for different aspects of their job); The incumbent reports to the Executive Director.

Externally: Successful and effective relations with patients community partners Chiropody community of practice and other clients that present themselves via telephone in writing or in person.
