Advanced Practice Provider (NP or PA) SNF Service Internal Medicine & Geriatric Care
Location: Twin Falls Idaho St. Lukes Clinic Post-Acute Medical Group

Key Responsibilities:
- Provide care through clinical judgment diagnostics and treatment planning
- Manage acute and chronic conditions in the SNF setting
- Maintain clear complete documentation to support continuity and quality of care
- Participate in team meetings peer reviews and continuous learning initiatives
Qualifications:
- Graduate of an accredited NP or PA program; eligible for Idaho licensure
- DEA and prescriptive authority (or ability to obtain)
- Strong interest in internal medicine geriatrics and transitional care
- Excellent communication and collaboration skills
