Job Description
Ohels Project is seeking Case Managers to work with Older Adults living in the Community. With the goal of older adults aging in place the candidate will provide case management services referrals to appropriate community connections and resources as well as supportive counseling.
Responsibilities:
- Perform in-home assessments and reassessments of homebound older adults and developcomprehensive care plans
- Provideextensive andongoing case management services including information caseassistance entitlement counseling referrals supportive contact advocacy and crisisintervention
- Keepdetailed and comprehensiverecords of all contact with clients including case filesand unit entries in thedatabase
- Attend trainingsas required including mandatoryNYC Agingtrainings
Required Qualifications:
- BachelorsDegree in Health-related field required Masters preferred
- Strong interpersonal clinicalorganizational skills as well as experience with benefits entitlements and resource applications
- Experience in working with the older adult population
- Bilingual Creole or Russian speaking preferred
The ideal candidate is a self-starter who can work independently as well as within a team and has with excellent follow up skills. This Full time position is located in Brooklyn offers a hybrid schedule with home visits as needed. Position includes a competitive benefits package.
Salary: Bachelors $50000- $55000
Masters $60000 - $65000
