DescriptionThe Medical Records Clerk organizes participant medical records reviews medical records for accuracy and completeness. Scan/file records into electronic or chart formats relying on technical skills and knowledge of commonly used concepts practices and procedures. Performs quality review to assure quality of legal electronic medical record and accuracy of placement in correct file. Relies on instructions and pre-established guidelines to perform the functions of the job.
Responsibilities- Medical Records Preparation/Scanning
- Applicable medical records are accounted for within established timelines and according to appropriate regulations.
- Preps medical records by verifying content organizing records into appropriate order and removing any items (such as staples clips etc.) that would hinder the scanning process.
- Applicable medical records are Quality Checked and Validated into electronic medical record system within established timeframes.
- Quality Review
- Verifies that scanned records are legible clearly scanned and contain all appropriate information. Structures scanning process to ensure records are placed into correct participants file and performs self-checks and audits to verify results.
- Medical records are validated with error rate held within acceptable limits.
- Batch Manager is monitored daily to assure all validated records are moving to the appropriate electronic medical record and other applicable parties are notified appropriately when records are not moving appropriately.
- Notifies manager and/or technical support of barriers or problems with systems that impact qualify of electronic medical record or efficiency and effectiveness of process.
- Release of information
- Responds to requests for medical records from other facilities within established timeframes and according to applicable regulations and guidelines. Confirms that requests contain appropriate approval and authority before releasing personal information. Maintains working knowledge of and adheres to all applicable HIPAA laws without fail.
- Assists with reaching out to other providers for requests for medical records needed by PACE. Sends request for information to these offices ensuring that requests contain all appropriate approvals to avoid unnecessary delays; follows-up with phone calls to ensure accuracy and timeliness of receipt of reports.

Qualifications- 1 year experience with medical records
- High School diploma or equivalent
- Organizational and communication skills
