Home Health Physical Therapist Opportunity:
- Full-time permanent direct-hire
- Schedule: Day shift 5x8 or 4x10
- In-home patient visits are required for the Home Health Physical Therapist.
Home Health Physical Therapist Responsibilities:
- Evaluate mobility strength balance and functional abilities to determine care needs.
- Develop and implement individualized treatment plans in collaboration with the patient family and care team.
- Provide skilled goal-oriented physical therapy with ongoing assessment and plan adjustments.
- Document care per regulatory guidelines and coordinate with interdisciplinary teams.
Home Health Physical Therapist Qualifications:
- Masters degree from an accredited Physical Therapy program
- Valid PT license or eligibility to obtain
- BLS Certification or eligibility to obtain
