Nurse Practitioner Post-Acute Care Flexible Schedule Killeen TX

Trumen Physicians Associates is seeking a compassionate and experienced Nurse Practitioner to join our expanding post-acute care team in Killeen TX. About the Role:

As a Nurse Practitioner in post-acute care you will provide in-person care to patients in skilled nursing and rehabilitation facilities. You will play a critical role in managing both chronic and acute conditions participating in transitional care planning and supporting the facilitys quality improvement initiatives.

Core Responsibilities:

  • Collaborate with Trumen physicians and facility medical directors to manage patient care

  • Conduct comprehensive clinical assessments and manage treatment plans

  • Participate in transitional care planning and prevent avoidable hospital readmissions

  • Reconcile medications and provide patient education

  • Foster positive working relationships with facility leadership clinical staff and care coordinators

  • Participate in Trumens value-based care programs by tracking patient outcomes and supporting quality improvement initiatives

  • Serve as a liaison between Trumen and assigned skilled nursing facilities (SNFs)

  • Maintain accurate and timely documentation within the EMR

  • Be present on-site during regular business hours depending on availability

Qualifications:

  • Active Nurse Practitioner license in the state of Texas

  • Minimum of 1 year experience as a Nurse Practitioner preferably in a post-acute SNF or geriatric care setting

  • Ability to work independently while effectively collaborating with the care team

  • Strong interpersonal skills and a passion for caring for complex patients

  • Reliable transportation within the Killeen and surroundingareas
