Job Description

Caretech Wyoming - Licensed Practical Nurse (LPN)

Location: Casper Wyoming

Position Type: Part-Time and PRN/Opportunity to Grow into Full-Time Hours

Reports To: Director of Nursing/Director of Operations

Position Summary:

Caretech is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to support our in-home care clients by providing skilled nursing services overseeing caregiver performance and ensuring each clients care plan is being followed and adjusted as needed. This role is ideal for a nurse who is passionate about both hands-on care and care coordination and who values meaningful relationships with clients and their families.

Key Responsibilities:

  • Provide in-home skilled nursing care as needed including wound care medication management assessments and other delegated nursing tasks.
  • Monitor client health and safety and assess for changes in condition intervening appropriately and updating care plans as needed.
  • Oversee and support caregivers by ensuring they follow care plans maintain professional standards and communicate client concerns effectively.
  • Conduct supervisory visits caregiver check-ins and quality assurance visits as required.
  • Collaborate with families physicians and other healthcare providers to coordinate care and advocate for client needs.
  • Assist with training and onboarding of new caregivers as needed.
  • Maintain accurate and timely documentation in compliance with Caretechs policies and state regulations.
  • Participate in on-call rotation and respond to urgent client or caregiver needs when required.

Qualifications:

  • Current and active LPN license in the state of Wyoming
  • Minimum of 1 year of nursing experience preferably in home health long-term care or a related field
  • Strong communication leadership and critical thinking skills
  • Experience working with caregivers or managing care teams preferred
  • Ability to work independently in the field and manage multiple client needs
  • Valid drivers license reliable transportation and willingness to travel locally
