Independent Contractor (1099) position Enjoy autonomy and tax flexibility
As a Post-Acute Care Physician youll manage a panel of patients in skilled nursing and rehabilitation facilities. Youll play a key role in reducing hospital readmissions managing chronic and acute conditions and guiding patients through transitional care with compassion and continuity.
Provide in-person medical care to patients in assigned post-acute facilities
Perform assessments manage treatment plans and update documentation in the EMR
Lead or collaborate with facility teams to ensure quality outcomes and smooth transitions of care
Participate in Trumens value-based care and transitional care programs
Develop strong working relationships with facility staff and leadership
Serve as a liaison between Trumen and facility teams
Be available in facilities during regular business hours (MondayFriday)
MD or DO with a current unrestricted Texas medical license
Board Certified or Board Eligible in Internal Medicine or Family Medicine
1 year of experience in post-acute geriatric or SNF care preferred
Comfortable practicing independently in skilled nursing facilities
Strong clinical judgment communication skills and a patient-centered mindset
Reliable transportation within the Bryan/College Station area
