Job Summary
The Behavioral Health Home Hospital (BHH) Research Psych Nurse (RN) will support an innovative care delivery model Behavioral Health Home Hospital. The program offers a subset of patients an alternative to standard inpatient psychiatric hospital admission in which patients can receive highly specialized psychiatric-care services in their home. The BHH Research Psych RN will provide quality clinical services to patients in various settings in the community. Patient care is delivered within organizational philosophy policy and standards of nursing and community health practice. The responsibilities are performed in accordance with Brigham and Womens Faulkner Hospitals policies and standards of practice utilizing thorough and timely electronic documentation and interdisciplinary communications. The BHH Research Psych RN is responsible for delivering patient care through the nursing process of assessment diagnosing teaching planning implementation and evaluation of BHH patients. The BHH Research Psych Nurse can expect to see patients twice a day in person in their homes or space they are residing in during their hospitalization. Additionally the BHH Research Nurse can expect to have up to 4 patients on service at a time. The BHH Research Psych Nurse directs and coordinates all nursing care for patients based on established clinical nursing practice standards and collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. The BHH Research Psych Nurse will be providing hospital/inpatient-level treatment to patients in the community. This is an ideal role for someone interested in a temporary position and working on a research trial that will help broaden the treatment opportunities for patients with active acute-psychiatric conditions.Qualifications

What youll do/Key Responsibilities
Plans and Implements treatment to patients in the community independently in collaboration with a multidisciplinary psychiatric team.
Accurately assesses patients needs.
Initial assessments and visit notes accurately document provision of nursing clinical care patient education and comprehension to ensure high quality patient care in accordance with program standards and regulatory compliance as applicable.
Patient schedule is established in collaboration with others. Schedules are routinely adjusted and communicated timely and in collaboration with the patients home-based clinical care team and is communicated with the patient and/or families to meet clinical care and program needs.
Utilizes multiple data sources to determine actual/potential health problems and functional status.
Utilizes Recovery Oriented Framework to identify patient strengths.
Performs point of care testing.
Patient information and change in status/plan of care is prepared and communicated to appropriate care team members and others as needed via effective communication methods.
Identifies patient behaviors that contribute to the development of a crisis; utilizes non-verbal and verbal techniques to de-escalate crisis situation.
Accurately plans nursing interventions for assigned patients
Plan of care is developed that adheres to patient needs safety and diagnosis for optimal patient outcomes. Patients and their families are actively included in the care planning process.
Teaching is provided from time of admission and evaluated on an ongoing basis to meet the needs and abilities of patients and caregivers learning styles and is appropriate to the care and treatment provided.
Discharge planning (that includes the patient family and multidisciplinary clinical care team and accesses community resources) is initiated on admission and evaluated on an ongoing basis for optimal patient safety and outcomes.
Demonstrates ability to prioritize patient care needs and partners with patients/families to support recovery goals.
Provides and implements nursing care interventions
Establishes a therapeutic alliance with assigned patients.
Medications are reconciled accurately and timely at admission discharge and transfer and any changes are integrated into the patients care plan and documented in the electronic health record.
Referrals are made to other disciplines and to appropriate community resources to maximize patient safety and outcomes.
Promotes interventions that provide the least-restrictive alternatives and recovery oriented goals.
Staff meetings competencies mandatory compliance training and educational meetings are attended regularly. Clinical initiatives and special training is supported.
Provides a leadership role in organizing and providing patient care.
Provides direction and supervision of MHSs.
Delegate care interventions.
Assumes a leadership role in medical and psychiatric emergencies.
Promotes/provides for critical incident de-briefing following emergency event.
Anticipates acute milieu problems and delegates staff appropriately.
Participates in the training and education of RNs MHSs and other healthcare providers.
Participate in staff meetings competencies mandatory compliance training educational meetings and research study trainings..
Patients are identified for multi-disciplinary case conference/rounding as needed.
Software is updated by maintaining the latest version of software applications as defined by the IS department.
Medical supplies and equipment are ordered according to allowable guidelines.
