Position: Outreach Case Manager Cradles Program
Reports to: Cradles Program Manager
Hours: Full time with some weekend and evening hours
Travel: Locally Required (Mileage Reimbursement Provided)
Status: Non-Exempt

The Role
The Outreach Case Manager (OCM) provides community-based intervention outreach services and evidence-based education to parents with children under the age of six OR expecting parents who have one or more risk factors for substance use.
The OCM works one-on-one with parents to evaluate needs and risks develop individualized strength-based service plans offer case management services and create space for families to engage in fun healthy sober free experiences.

Bachelors degree in Social Work or other related field or LCDC-I or LCDC (strongly preferred)
Minimum of two years in the field of Behavioral Health to include substance use experience
Minimum of two years case management experience
Previous experience working with vulnerable families/teens in the substance use field
Working knowledge of client/community resources and experience in developing partnerships
Experience leading groups with vulnerable populations
Experience working independently and collaboratively as part of a team Valid drivers license automobile insurance and reliable transportation
Bilingual (preferred)
