Description

JOB SUMMARY: Oversees therapeutic services and the CarePartner team. Responsible for developing coordinating implementing and facilitating recreational and therapeutic activities that enhance the well-being of program participants and their ability to interact socially and maintain activities of daily living. This position requires a working knowledge of and compliance with State and Federal regulations governing PACE organizations.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Supervision

  1. Facilitates and supports an effective team approach in implementing PACE guiding principles. Works with management to foster a positive team environment.
  2. Delegates mentors and supports the CarePartners in activities PT/OT maintenance and meal services.
  3. Directly supervises CarePartners at the assigned program facility. Assists the Program Director in personnel responsibilities related to recruitment interviewing staffing and evaluation of Day Center CarePartners and activity program employees.
  4. Guides competency evaluations program-orientation ongoing training and performance coaching for CarePartners. Completes the annual evaluations and bi-annual goal reviews for employees supervised.
  5. Oversees volunteer recruitment orientation training and recognition.
  6. Acts as a leader and mentor in implementation of Person-Centered Care.
  7. Reports to the Center Manager concerning operations of the Day Center including situations or occurrences that impact safety and efficiency.
  8. Facilitates and supports an effective team approach in implementing the programs guiding principles. Attends and participates in the Inter-Disciplinary Team (IDT) management meetings and other team meetings as requested. Facilitates daily huddles with program employees.
  9. Supports a person-centered approach to management staffing and program implementation.

Leadership

  1. Oversees overall day center activities and services.
  2. Trains CarePartners in individual and group crafts games and discussion groups as well as planning field trips.
  3. Facilitates training provided by the centers Physical Therapist and Occupational Therapist for CarePartners and Therapy Aides in the implementation of group and individual PT & OT Maintenance Plans.
  4. Facilitates training for CarePartners in the provision of personal care services (Activities of Daily Living).
  5. Participates as an active member and leader of various department and team meetings including CarePartner team meetings and Interdisciplinary Team (IDT) meetings.
  6. Attends in-service trainings and presents training topics to program team members and volunteers as requested by management.

Participant Care

  1. Works with elderly and disabled adults to support their participation in a variety of activities at the Day Center.
  2. Evaluates the needs abilities and interests of each participant and ensures the development of an individualized plan of care that reflects the principles of Person-Centered Care.
  3. Collects and considers input from team members and participants about the monthly calendar of activities. Plans for implements posts and distributes monthly calendar of activities.
  4. Facilitates quarterly participant advisory meetings and reviews individual plans of care to monitor the quality of services.
  5. Complies with all policy and procedures of Redwood Coast PACE.

Program Development

  1. Evaluates on a regular basis through participant interviews and team operations meetings participants changing needs. Facilitates implementation of necessary activity program adjustments.
  2. Researches and implements activities appropriate for specific participant populations. Ensures provision of activities specified in the participants individualized plans of care.
  3. Completes necessary paperwork and documentation to ensure compliance with applicable regulations.
  4. Works with Center Manager to purchases activity and day center supplies.
  5. Maintains storage of supplies and supply areas.
  6. Assists as needed in the set-up and cleanup of activity supplies. Moves tables chairs and activity equipment for different activities.

Community and Intra-Agency Interactions

  1. Networks and coordinates with community agencies as appropriate.
  2. Assists with fundraising activities on behalf of the program and agency including participation in special events and fundraising for specific program purposes.
  3. Attends agency committees as requested by the Department Director.
  4. Networks and coordinates with community agencies as appropriate.
  5. Acts in a manner consistent with agency values.

Qualifications

EDUCATION &/or EXPERIENCE:

1. High school diploma or GED equivalent required.

2. The Activity Supervisor will have one of the following:

  • Two (2) years of experience in a social recreational or educational program within the past five (5) years one (1) year of which was full-time in an activities program in a health care mental health or handicapped program setting;

or

  • Be an occupational therapist art therapist music therapist dance therapist recreation therapist occupational therapy assistant or a qualified social worker;

or

  • Have completed at least 36 hours of training in a course designed specifically for the activity coordinator role and approved by the State department of health services. Will receive regular consultation from an occupational therapist qualified social worker or a recreation therapist.

3.Candidates with a bachelors degree in kinesiology exercise science or other therapy-related discipline preferred.

4. One (1) year of experience working with a frail or elderly population providing personal care in a health facility or in-home setting preferred.

LICENSES OR CERTIFICATES:

  1. Current CPR and First Aid certification within six (6) months of hire.
  2. One of the following Certified designations preferred: Therapeutic Recreation Specialist Physical Therapy Aide Nursing Assistant Home Health Aide.
  3. Valid California Drivers License and personal auto liability insurance preferred. Personal vehicle in good driving/operating condition insured for State minimum liability requirements that can be used for the applicable job functions noted above preferred.

PRE-SCREENING REQUIREMENTS:

  • Requires clearance of a DOJ and FBI criminal history background check
  • Pre-employment physical and PPD skin test.
  • Requires Motor Vehicle Report verification.
  • COVID-19 and Influenza (flu) vaccines preferred.

  6. Demonstrates ability and willingness to expand knowledge regarding relevant developments in the field of activities.
  7. Demonstrates interest in working with elderly and disabled adults to enhance and encourage their independence.
  8. Basic knowledge of computers and web-based applications such as timekeeping training and email programs.

SUPERVISORY REQUIREMENTS:

  1. This position supervises the following employees: Center CarePartners
  2. In coordination with Human Resources and other supervisory staff oversees hiring orienting training evaluating disciplining and terminating of specified departmental staff.

PHYSICAL REQUIREMENTS:

  1. Physical abilities sufficient to move between different staff work areas communicate with co-workers and the public operate a computer produce reports talk on the telephone and travel to other agency worksites.
