This is an opportunity for a mission-driven leader who is passionate about ensuring that operations in our PACE center run smoothly for our participants. The Operations Manager at the PACE Center oversees the daily operations of our teams providing transportation coordinating external appointments and maintaining physical plant. They are an expert at defining and implementing operational processes and tools that our team can carry out. The Operations Manager provides leadership to their direct reports while collaborating with fellow leaders to ensure participants physical social emotional and recreational needs are met. This position ensures high-quality person-centered care in alignment with PACE standards and regulatory guidelines.
Core Responsibilities & Expectations for the Role
- Contribute to a center experience that Participants want to spend time in a team culture that cares and creates joy and an environment where all participants and team members belong.
- Continue to raise the bar. Constructively seek and share feedback and help us implement changes in order to improve clinical outcomes and experience for participants.
- Exhibit and honor Habitats values.
- Supervise one or more referral coordinators in order to ensure our participants have a positive and reliable experience utilizing outside specialists and those outside specialists have a positive experience of Habitat Health.
- Oversee our transportation coordinator and drivers ensuring timely and effective transportation for our participants.
- Project manager for Quality Improvement efforts onsite often taking a lead role in process redesign.
- Accountable for ensuring clear lines of communication between departments troubleshooting any communication gaps and redesigning workflows or delivering training to correct.
- Develop and maintain staffing schedules and assignment of duties.
- Collaborate with the interdisciplinary team (IDT) to assess participant needs and update care plans.
- Support a culture of safety dignity and respect for all PACE participants and staff.
- Performs related duties as assigned.
Required Qualifications:
- Bachelors Degree
- Experience leading process design or quality improvement initiatives.
- Knowledge of PACE program or similar healthcare operations.
- Ability to work without close supervision or professional guidance and to exercise independent judgment.
- Knowledge of physical mental and social needs of frail older adults.
- Ability to be self-directed and able to work independently.
- A minimum of two years in a supervisory position.
- Proof of medical clearance for communicable diseases including a TB test.
- Proof of all immunizations are up to date.
- Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment.
- Proof of valid CA drivers license personal transportation good driving record and auto insurance as required by State law.
Preferred Qualifications:
- Bilingual: Spanish/Mandarin/Cantonese preferred.
- A state issued drivers license personal transportation and auto insurance as required by law.

Compensation:
We take into account an individuals qualifications skillset and experience in determining final salary. This role is eligible for medical/dental/vision insurance short and long-term disability life insurance flexible spending accounts 401(k) savings paid time off and company-paid holidays. The expected salary range for this position is $123000 - $141000 and is bonus eligible. The actual offer will be at the companys sole discretion and determined by relevant business considerations including the final candidates qualifications years of experience skillset and geographic location.
