Center for Autism and Related Disabilities (CARD) Department of Psychology College of Arts & Sciences has an open Case Manager position to work with adolescents and adults with autism and related disabilities. Excellent interpersonal communication skills compassion time management organization and problem solving skills are a must. Expected to have good public speaking skills and a dynamic presentation style.

The Case Manager coordinates the overall interdisciplinary plan of care for patients from admission to discharge. The Case Manager provides ongoing support and expertise through comprehensive assessment planning implementation and overall evaluation of individual patients needs. The incumbent acts as a liaison between patients families and healthcare personnel to ensure necessary care is provided promptly and effectively.

Department Specific Responsibilities:

25% - provides direct individual family assistance to eligible transition age CARD constituents. Consistently exercises discretion and judgment in analyzing interpreting and determining a proper course of action and treatment plan that best serves client needs. Works closely with the client/family demonstrating good judgment and discretion to ensure fidelity of implementation of the treatment plan. Works closely with client/family to re-calibrate treatment course if necessary.

20% independently plans and facilitates a Self-Determination group for middle and high school students with autism. Develop and implement session activities that equip students with essential skills to advocate for their needs set and achieve goals and actively participate in their education. Incorporates strategies for emotional regulation and coping to support students in building confidence and independence.

15% - develops and facilitates cross-training for staff providing individualized family assistance to eligible transition age CARD constituents. Creates materials to guide staff in assessing and evaluating transition documentation.

15% - partners with Project 10 Regional Transition Representative to plan organize and facilitate quarterly transition workshops aimed at supporting students and families in navigating transition. This role involves developing and distributing comprehensive workshop materials that clearly outline the presentations key topics and concepts while ensuring all content aligns with current policies and available resources in transition planning.

10% - collaborates with local middle and high schools to identify opportunities to attend transition nights and offers on-site training for families of transition age students.

5% - independently writes and maintains case reports electronic client records and other professional correspondence in accordance with applicable standards and regulations and maintains a high level of ethical conduct regarding client confidentiality.

5% - participates in case conferences staff meetings and quarterly regional full staff meetings. Exchanges pertinent information with supervisor and receives supervision as needed.

2% - maintains a network of appropriate referrals related to the needs of transition age clients and independently makes appropriate referrals for clients and families as part of the regular duties.

2% - maintains contact data case notes training summaries and evaluations and other records in accordance with statewide CARD uniform reporting guidelines.

1% - other duties as assigned

MINIMUM QUALIFICATIONS


Education:
Bachelors degree in special education psychology behavior analysis social work or related field.

Experience:
Minimum 3 years of relevant experience required.
