If you feel strongly about advocacy and empowering individuals with disabilities in Utah this job is for you. The Division of Integrated Healthcare within the Department of Health and Human Services is looking for a dedicated professionals to provide education and in-reach. The person in this position will provide education and in-reach for people with disabilities who reside in intermediate care facilities. This will include organizing and conducting group trainings and/or workshops meeting regularly with members and families individually developing program materials and serving as the primary contact for individuals and families regarding Home and Community Based Services.Note: as of April 8 2024 DHHS employees will be working a hybrid schedule which includes 2 office days per week and 3 telework days.

Responsibilities
- Assist people with disabilities through successful transition from Intermediate Care Facilities to Home and Community Based Services.
- Develop education materials for clients families and other stakeholders.
- Provide education to individuals and families regarding home and community based services and available service options.
- Provide experiences to assist individuals in making informed choices.
- Conduct and facilitate group trainings workshops or other activities
- Ongoing monitoring of education/in-reach program to identify areas of improvement.
- Program data tracking evaluation and reporting.
- Other related duties as assigned
Qualifications
Minimum Qualifications:
- 12 months of direct work with persons with intellectual/developmental disabilities
- Working knowledge of Home and Community Based Services (HCBS)
- Knowledge of HCBS Settings Rule
- Strong interpersonal skills
- Passionate about empowering people with disabilities
- Excellent listener who is also very proactive
- Ability to prioritize multiple tasks while maintaining attention to detail
- Knowledge of community resources for people with disabilities and how to access those resources
- Excellent communication skills with ability to speak in front of large groups
- Self-motivated with ability to work a flexible schedule
Supplemental Information
- Risks found in the typical office setting which is adequately lighted heated and ventilated e.g. safe use of office equipment avoiding trips and falls observing fire regulations etc.
- Typically the employee may sit comfortably to perform the work; however there may be some walking; standing; bending; carrying light items; driving an automobile etc. Special physical demands are not required to perform the work.
- Typical work schedule is Monday - Friday 8:00 am to 5:00 pm.
- This position may currently be a hybrid of both in-office and remote work days. Please note a positions eligibility for remote work is established by agency management and is subject to change at their discretion at any time and for any reason.
EffectiveJuly 1 2021 State of Utah Employees must either reside in Utah or be in the process of relocating to Utah within 30 days of their start date.
