Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and potentially hospice care to patients of varying ages and health conditions. The RN will perform patient assessments administer medications develop treatment plans and collaborate with an interdisciplinary team. A minimum of two years of experience in a relevant setting is required.
Location: Oregon City Oregon United States
Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and support staff.
- Case management and care coordination.
- Providing skilled nursing care in various settings (potentially including Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room).
- Charting patient information (using Epic system).
Required Skills & Certifications:
- Active Registered Nurse (RN) license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of experience in hospital home health or hospice care.
- Clinical assessment medication administration and treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and personal vehicle for field work (if applicable).
Preferred Skills & Certifications:
- Experience in multiple specialties (Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room).
- Experience with hospice care.

- On-call requirements: Yes 100 hours per 4-week schedule.
- Weekend work: Yes on-call.
- Floating between units: Yes.
- Patient ratios: 1:1.

- Short-term acute care hospital setting.
Scheduling:
- Schedule determined by the employer.
- On-call rotation.
- Weekend on-call shifts required.
