Job Summary:
This Registered Nurse position requires providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration skills are essential. A minimum of two years of experience is required.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and healthcare professionals.
- Case management and coordination of care.
- Providing skilled nursing care in various settings (potentially including Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room).
- Hospice care.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and healthcare professionals.
- Case management and coordination of care.
- Providing skilled nursing care in various settings (potentially including Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room).
- Hospice care.
Required Skills & Certifications:
- Clinical assessment skills.
- Medication administration skills.
- Treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of hospital home health or hospice care experience.
- Drivers license car insurance and automobile for field work.
- Medication administration skills.
- Treatment planning skills.

- Active Registered Nurse license in the state of employment.
