Key Responsibilities
Comprehensive Patient Care
Conduct comprehensive mental health assessments and diagnostic evaluations.
Develop implement and revise individualized treatment plans.
Deliver outpatient therapy services to adults and/or children.
Provide crisis intervention and behavioral stabilization when needed.
Team Collaboration & Community Engagement
Collaborate with care teams including physicians case managers and social workers.
Build partnerships with community organizations to strengthen patient support systems.
Refer and connect patients to appropriate public and mental health resources.
Monitoring & Reporting
Maintain accurate and timely clinical documentation treatment notes and discharge plans.
Track patient progress through ongoing assessments and adapt interventions accordingly.
Prepare case summaries and reports for internal and external stakeholders.
Qualifications
Education & Licensures
Experience
Minimum of 3 years experience working with diverse mental health populations.
Skilled in conducting assessments developing treatment plans and providing therapy.
Background in crisis management and behavioral interventions.
Skills & Competencies
Strong foundation in evidence-based practices and psychosocial care.
Proficient in crisis response and emergency decision-making.
Effective communicator with strong counseling and collaboration skills.
Knowledge of community-based mental health resources and patient advocacy.

Education
Master s Degree in Social Work, Counseling, or a related field.
