Nurse Practitioner (NP)
Titan Placement Group invites you to explore an opportunity in Richmond Virginia.

Our client is a physician-led value-based care provider dedicated to transforming healthcare for senior populations especially those managing multiple chronic conditions.
Salary and Benefits
- Base Salary: $145000 annually
- Bonus: Up to $10000 in the first year with additional potential as a partner
- Comprehensive benefits package including:

- 401(k) with 5.5% match
- Work Schedule:
- Monday through Friday 7:30 AM 5:00 PM (last patient at 4:15 PM)
- No weekends or nights
- Dedicated administrative time built into the schedule
- Additional Perks:
- Small patient panel (max 450)
- Transportation provided to patients at no cost
- Dedicated medical assistant scheduler and care team support
- In-house specialists including cardiology and podiatry
- Holistic services such as acupuncture Tai Chi yoga and nutrition
Responsibilities
- Deliver outpatient primary care to a senior patient population
- Manage 18 patients per day with a focus on quality not quantity
- Provide chronic disease management for 35 conditions per patient
- Collaborate with a multidisciplinary care team to support patient outcomes
- Practice under a value-based care model to drive better health results
- Document care and outcomes effectively in electronic health records
- Participate in partnership and growth opportunities within the organization
Requirements
- Board Certification in Family Medicine or Geriatric Medicine
- Active Nurse Practitioner license in the state of Virginia (or ability to obtain)
- Experience in primary care geriatrics or value-based models preferred
- Passion for working with underserved or senior populations
- Commitment to compassionate team-based care
