Job Summary:
This role involves providing skilled nursing care case management and hospice care to patients facing advancing age frailty and serious illness. The Registered Nurse will perform patient assessments administer medications develop treatment plans and collaborate with the interdisciplinary team. Strong communication and collaboration skills are essential. On-call responsibilities are expected 1-2 days per week including rotating weekend coverage.

- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- On-call responsibilities are expected 1-2 days per week and rotating weekends.
- Acute care hospital setting.
