DescriptionThe Community Health Outreach Coordinator plans and supervises the implementation of community health programming. The Coordinator takes responsibility for all metrics related to Community Outreach; also takes lead on collaboration and partnerships throughout the hospital hospital system and the community. The Coordinator manages all aspects of outreach and the staff resources to accomplish this including hiring scheduling and supervising staff to meet department goals.

- Project Management
- Develops coordinates and implements community events and evaluates program effectiveness.
- Prepares and distributes information throughout the family of services; provides monthly event schedules to all.
- Manages and coordinates all volunteers associated with community events for screenings as well as manages and evaluates college interns. Maintains relationship with college departments in order to recruit and supervise internships and service learning partnerships- Distribution of membership cards.
- Leads efforts and workgroups related to Community Health Needs Assessment.
- Supports the Parish Nursing program managing the relationship offering new screenings introductions to vendors and new resources for their outreach efforts and annual health fairs.
- Data Management/Reporting
- Analyzes information gathered from assessments and offers comprehensive wellness offerings as needed.
- Collaboration and Partnership
- Serves as a resource to marketing business development and the community by offering support services. Also serves as informational liaison by linking to community resources and partners.
- Develops positive relationships between the hospital and community agencies special interest and civic groups and professional affiliations by acting as a hospital representative. Works through these relationships and public education activities to improve community health and foster the hospitals image as a leader in the health care industry.
- Maintains a relationship with nonprofit organizations to engage team members in community services to serve those most in need.
- Works closely with partners to incorporate cancer screenings into our normal outreach efforts.

QualificationsExperience - 3 years experience working in Community Health programming or related field
Education - Bachelors degree in Health Promotion or related area of study
Licensure - BLS Certification
Required Experience:
