Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate effectively with the interdisciplinary healthcare team. A minimum of two years of experience is required.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and allied health professionals.
- Case management and coordination of care.
- Providing skilled nursing care in various settings (hospital home health or hospice).
- Maintaining accurate and timely documentation.
- Adhering to all hospital policies and procedures.

- BLS certification.
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.

- Must be able to work in a fast-paced acute care hospital environment.
