Job Summary:
This role involves providing skilled physical therapy assistance in a home health setting. The Physical Therapist Assistant (PTA) will treat patients in their homes managing a caseload of up to six patients daily focusing on rehabilitation and recovery. The position requires adherence to established productivity standards and utilizes electronic charting systems. A valid drivers license and reliable transportation are essential.

Responsibilities:
- Providing direct patient care in a home health setting.
- Managing a caseload of up to six patients per day.
- Performing patient assessments and treatment planning.
- Charting patient progress using electronic health record systems (EPIC and Rover/Oasis).
- Maintaining accurate documentation and adhering to productivity standards (30 productivity points).
- Collaborating with the interdisciplinary team.
- Traveling to patient homes within a designated geographic area (up to 60 miles radius).
- Maintaining compliance with all relevant regulations and policies.
Required Skills & Certifications:
- Associates degree in Physical Therapist Assistant.
- One year of skilled home health experience.
- Current Basic Life Support (BLS) certification.
- Active PTA license in the state of practice.
- Proficiency in Oasis electronic health record system.
- Valid drivers license reliable transportation and auto insurance.
- COVID-19 vaccination (primary series and one booster).
Preferred Skills & Certifications:
- Experience with EPIC electronic health record system.
Special Considerations:
- 8-hour day shifts; no 4x10s or 3x12s accommodated.
- No weekend requirements.
- No on-call responsibilities.
- No floating between units.
- Block scheduling not accommodated.
- Mileage reimbursement at the IRS standard rate.
- Facility may call off up to one shift per pay period.
- Business casual attire; scrubs allowed but not required.
- Occasional maximum drive time of 60-90 minutes per day.
