Job Summary:
This position is for a Physical Therapist Assistant providing skilled home health services to patients in the West Portland OR and surrounding areas. The PTA will provide direct patient care adhering to established protocols and collaborating with the interdisciplinary team. The role requires a minimum of one year of skilled home health experience and proficiency in Oasis charting.

The PTA will provide direct patient care adhering to established protocols and collaborating with the interdisciplinary team. The role requires a minimum of one year of skilled home health experience and proficiency in Oasis charting.
Location: Portland Oregon United States
Responsibilities:
- Provide direct patient care in accordance with established treatment plans.
- Perform physical therapy interventions as prescribed by the supervising physical therapist.
- Document patient progress and treatment sessions accurately and completely in the electronic medical record (EPIC and Oasis).
- Communicate effectively with patients families and other members of the healthcare team.
- Manage patient schedules and travel efficiently within a defined geographic area.
- Maintain compliance with all relevant regulations and standards.
- Utilize Rover (phone app) for communication and scheduling.
- Adhere to business casual dress code.

Required Skills & Certifications:
- Associates degree in Physical Therapist Assistant.
- Current Physical Therapist Assistant license in Oregon.
- Minimum of one year of skilled home health experience.
- Basic Life Support (BLS) certification.
- Proficiency in Oasis electronic health record system.
- Valid drivers license reliable transportation and auto insurance.
- COVID-19 vaccination (primary series and one booster).
- Current Physical Therapist Assistant license in Oregon.
- Minimum of one year of skilled home health experience.
- Basic Life Support (BLS) certification.
- Proficiency in Oasis electronic health record system.
- Valid drivers license reliable transportation and auto insurance.

Preferred Skills & Certifications:
- Experience with EPIC electronic health record system.
Special Considerations:
- 8-hour day shifts; no 4x10 or 3x12 shifts accommodated.
- Up to 6 patients per day 30 productivity points standard.
- No weekend requirements.
- No on-call responsibilities.
- No floating between units.
- No block scheduling accommodated.
- Occasional travel up to 60 miles radius (60-90 minute drive time).
- Facility may call off up to one shift per pay period.
