Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with an interdisciplinary team. A minimum of two years of experience is required.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration.
- Treatment planning and implementation.
- Collaboration with interdisciplinary team members (physicians other nurses therapists etc.).
- Case management and hospice care (as applicable).
- Maintaining accurate patient records.
- Providing skilled nursing care.
Required Skills & Certifications:
- Active Registered Nurse (RN) license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of experience in hospital home health or hospice care.
- Clinical assessment medication administration and treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and personal vehicle for field work (if applicable).
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- Weekend and holiday rotations are required.
- Rotating on-call responsibilities for weekends holidays and shifts.
- Floating between similar units within the facility may be required.
Scheduling:
- Shift type and scheduling details will be determined upon hire. Weekend and holiday rotation is required.
