Job Description
Overview
We are seeking a dedicated and experienced PRN Occupational Therapist to join our Home Health team and help senior patients regain independence and improve their quality of life.
The Occupational Therapist (OT) provides intermittent home-based therapy services as prescribed by a physician. The OT will assess develop and implement treatment plans aimed at improving functional skills and promoting optimal independence.

Responsibilities
- Administer prescribed occupational therapy treatments to senior patients in their homes.
- Conduct initial assessments and create individualized plans of care.
- Perform skilled procedures and maintain accurate clinical documentation.
- Monitor and report patient progress to physicians and adjust care plans as needed.
- Educate patients and caregivers on home programs and therapeutic exercises.
- Collaborate with interdisciplinary teams and participate in care planning.
- Supervise and guide support staff as required.
- Participate in agency in-services QAPI activities and promote agency mission.
Qualifications
- Must be a graduate of an accredited Occupational Therapy program.
- Must be a Licensed Occupational Therapist in Arizona.
- A minimum of two years of clinical experience is preferred.
- Strong interpersonal skills and documentation skills.
- Excellent communication skills and a passion for providing high-quality care.
Preferred Experience
- Previous home health care experience.
- Proficiency in upper extremity rehabilitation and activities of daily living (ADLs).
