Overview
Dignity Health Mercy San Juan Medical Center is a 370-bed not-for-profit Level 2 Trauma Center located in Carmichael California serving the areas of north Sacramento County and south Placer County for more than 50 years. It is one of the areas largest and most comprehensive medical centers. Dedicated to the communitys well-being our staff and volunteers provide excellence in care for our patients each year. Mercy San Juan Medical Center has received recognition for being a Comprehensive Stroke Center and Center of Excellence for Bariatric Surgery along with Certificates of Excellence in Perinatal Care Hip and Knee Replacement.
Responsibilities
The incumbent provides care treatment and service to Home Health and Hospice patients and their families in their place of residence within their scope of practice. Functions as a member of the interdisciplinary team.

Qualifications
Experience:
- Two (2) years LVN Experience within the last 5 years
- Recent experience as an LVN in a Home Health or Hospice Agency preferred.
Licensure:
- Currently licensed as a LVN in California
- Current California drivers license possession of automobile insurance & access to an operating motor vehicle.
- Current CPR/BLS card approved through the American Heart Association.
