Blanchard Valley Palliative Care is seeking a full-time Nurse Practitioner to join our compassionate and collaborative team providing outpatient hospice and palliative care services across Findlay Ohio and surrounding counties.

Provide outpatient palliative care services including home visits extended care facility consultations telehealth and clinic-based care.
Manage 68 patients per day based on location across a 9-county area in NW Ohio.
Participate in delicate goals-of-care discussions with patients and families facing serious or terminal illnesses.
Assess plan implement and evaluate individualized care needs in alignment with palliative care principles.
Collaborate with a supportive interdisciplinary team including other NPs physicians and administrative staff.
Participate in care transitions including determining when patients would benefit from continued palliative care or hospice services.
Work closely with a hospitalist team responsible for all patient admissions.
Document patient encounters thoroughly and maintain excellent communication with patients families and care teams.
