Job Description

  • Part Time Ongoing Contract
  • 32 hours per fortnight

Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute sub-acute aged care and community services.

We are seeking a dynamic and motivated Sub Acute Care Coordinator to lead patient-centred assessment and discharge planning for individuals admitted to our Sub Acute Rehabilitation and GEM this key role you will provide effective communication with patients and families establish SMART goals tailored to individual needs and ensure timely and coordinated discharges. You will manage a care coordination caseload support clients with complex needs and liaise with community services and stakeholders to deliver comprehensive care.

As a leader within the Multidisciplinary Team you will play an active role in mentoring staff enhancing care coordination capabilities and contributing to access and flow strategies across the unit.
