Job Description
Position: Physical Therapist - Home Health
Start Date: 8/18/2025
Duration: 13 weeks
Location: Grinnell IA
Shift Type/Shift Times/Hours/Day: Days
Shift Notes: Monday - Friday 4x8 hr shifts; 32 hrs/week
Weekend Requirements: None
On-Call Requirements: None

OT/Holiday Multiplier: 1.25x
NBO: 8 hrs
Floating Requirement:
- Must be willing and able to supporting the Grinnell (2-3x/week) and Des Moines (1-2x/week) regions
As a member of the interdisciplinary team contributes professional nursing knowledge and skills in the provision and management of care to patients through the application of the nursing process.
Submission Requirements- Home health experience is a plus
- at least 1.5 years of PT experience
- experience working with all age groups
PROFILE REQUIREMENTS:
- Work history/resume
- Home/taxable address or Drivers License
- Skills checklist (must have been completed within 1 year of submission date)
- License verification (IA PT license must be active)
- Copy of all certificates
- References - highly preferred (if not provided will be required to complete during onboarding if candidate accepts an offer)
