Were seeking a Nurse Practitioner Physician Associate or Physician in Boise ID to lead our Transitional Care Management (TCM) efforts with a unique opportunity to grow a dedicated primary care panel in assisted living communities youll come to know through the TCM process.
High-Impact TCM Care: Partner with top SNFs (including Ensign communities) to deliver timely post-discharge care reducing readmissions and improving recovery outcomes.
Relationship-Driven Primary Care: Seamlessly transition TCM patients into your own primary care panel in assisted living settings where trust and continuity flourish.
Autonomy & Flexibility: You manage your day we provide the infrastructure and support to help you succeed.
Growth-Oriented: Were building a next-generation care model that values providers as long-term partners not just contractors.
Perform timely TCM visits for patients recently discharged from skilled nursing facilities in the Boise area.
Build and maintain a primary care panel in local assisted living communities through trusted post-acute care relationships.
Manage acute and chronic conditions with a focus on prevention and patient engagement.
Coordinate with facility staff families and interdisciplinary teams to ensure seamless care transitions.
Serve as a reliable compassionate provider in both SNF and community-based settings.
Licensed NP PA or MD/DO in Idaho (or eligible for licensure)
Experience in transitional care geriatrics primary care or post-acute settings
Skilled communicator and relationship-builder with patients families and facility partners
Independent and self-motivated with a commitment to excellent care delivery
Passionate about improving healthcare for older adults in Boise
