What Youll Do
- Deliver compassionate high-quality nursing care tailored to each patients needs.
- Develop and adjust individualized care plans to ensure optimal patient outcomes.
- Collaborate with physicians therapists and other healthcare professionals.
- Supervise your caseload nurturing the growth and development of LPNs and Home Health Aides as we cultivate a team of caregivers.
- Provide education and support to patients and their families.
- Maintain thorough documentation to meet regulatory and operational standards.
- Utilize the latest technology and best practices to enhance patient care.

Qualifications
- Education: Graduate of an accredited nursing program; must hold an active RN license in the state served.
- Experience: At least 1 year of nursing experience (Home Health preferred).
- Skills & Abilities: Strong critical thinking clinical expertise case management and a passion for patient care.
- Additional Requirements: CPR/BLS certification
- Valid drivers license.
